Job summary

El Centro Family Health in Northern New Mexico is seeking a Referral Coordinator to support patients and providers in the referral process for specialty care. You will ensure continuity of care by following health-plan protocols and may provide psychosocial support as needed.

Responsibilities include submitting pre-authorizations, updating the EMR (eCW), and coordinating communication between patients, clinics, and outside providers.

Qualifications

• High school diploma or GED required.
• Associate or bachelor's degree in healthcare preferred.
• Bilingual English/Spanish is preferred.

Responsibilities

• Complete, verify, and submit records and pre-authorizations on time.
• Follow HIPAA and patient privacy rules.
• Answer questions, schedule appointments, and give clear instructions.
• Use eClinicalWorks EMR to update provider lists and log referrals.
• Send medical data to outside clinics and transfer calls to the right provider.
• Monitor referrals to ensure attendance and return of outside records.
• Contact patients who miss appointments and document reasons.
• Assist with other duties and travel as required.
