Job Responsibilities:
Telephonic delivery of patient support services
Serve as a liaison between patient, professional health care departments and Central Office management
Helping to guide patients in need of medical services.
Reviewing relevant information in the Electronic Medical Record
Collaborative Coordination of appointments for patients at H+H Facilities
Tracking the results of outreach calls made to patients
Understanding the value-based payment metrics driving patient outreach
Communicating with patients in a caring, empathic, and culturally competent manner
Job Requisites:
One year experience
Basic Excel skills
Patient or customer outreach/interactions
Strong interpersonal skills, both verbal and written.
Excellent customer service skills.
Must be able to type 25 to 35 wpm.
