Responsibilities
- Serve as the primary point of contact for patients, assisting them in navigating the healthcare system and coordinating their care.
- Provide education and counseling to patients about their diagnoses, treatment options, and available resources.
- Collaborate with interdisciplinary teams to develop, implement, and evaluate patient care plans.
- Facilitate communication between patients, their families, and healthcare providers to ensure patient needs are met and concerns are addressed.
- Advocate for patients' rights and needs, ensuring they receive appropriate care and services.
- Assist patients in understanding and managing their healthcare bills, insurance, and other financial matters.
- Conduct outreach to at‑risk populations, promoting HIV/STD prevention and treatment strategies, and harm reduction approaches.
- Maintain accurate and timely documentation of patient interactions, interventions, and outcomes.
- Participate in continuous learning and professional development activities to stay updated with the latest trends and best practices in patient navigation and healthcare.
Qualifications
- A minimum of 2+ years of experience in patient navigation, healthcare administration, or a related field.
- Experience in outpatient clinics, providing support to patients.
- Knowledge of HIV/STD treatment and prevention strategies, with a focus on harm reduction.
- Exceptional interpersonal and communication skills, with the ability to interact effectively with diverse patient populations and healthcare professionals.
- Strong advocacy and problem‑solving skills, with the ability to effectively coordinate care and resolve patient concerns.
- Proficiency in health information management systems and Microsoft Office suite.
- Ability to maintain patient confidentiality and adhere to ethical standards.
- Bachelor's degree in healthcare administration, nursing, social work, or a related field is preferred.
- Certification as a Patient Navigator or similar certification is a plus.
