Job summary

You will guide patients through the 26Health system, coordinate scheduling, referrals, and education, with a patient-centered approach and a focus on removing barriers to care.

Qualifications

  • High school diploma or equivalent is required.
  • Bilingual English/Spanish proficiency is required.
  • 2+ years in a related role within a healthcare organization is preferred.
  • Must complete a 20+ hour online training course and pass final exam for Federal certification prior to assuming duties.
  • Must be able to work flexible hours, including evenings and weekends.
  • Excellent verbal and written communication skills; open, trustworthy communication style.
  • Proficient in MS Office applications (Excel, Word, PowerPoint).
  • Experience working with ECW or other EHR systems is highly preferable.

Responsibilities

  • Performs work with patient care and superior service as focus.
  • Conducts customer service calls to ensure quality patient experience.
  • Schedules patients’ appointments and provides information for tests.
  • Refers patients to internal or external services as needed.
  • Identifies resources to overcome barriers to care (transportation, housing, health insurance).
  • Explains Marketplace enrollment and program options to clients.
  • Explains health package options to uninsured patients and facilitates enrollment.
  • Ensures payments are billed per contracts and schedules initial wellness exam.
  • Sends monthly patient satisfaction surveys and analyzes feedback for improvements.
  • Collaborates with Training & Education to enhance patient experience.
  • Performs other duties as required.

Job Purpose

The Patient Navigator serves as a key resource for clients, fostering ongoing engagement in medical care, improving communication, and facilitating access to social services. This role is responsible for guiding patients through the 26Health system with empathy and efficiency, removing barriers to care and supporting timely re-engagement in treatment. Through a patient-centered approach, the Patient Navigator ensures individuals receive the support, education, and resources they need to navigate their healthcare journey successfully.

Duties and responsibilities
  • Performs work from a position that patient care and superior service are the focus and priority in everything we do.
  • Conducts customer service calls to ensure quality patient experience.
  • Schedules patients’ appointments and provides information for individual tests.
  • Refers patients to inside services (i.e.,medical patients to mental health, or HIV at-risk patients to PrEP , substance abuse counseling) or outside services as dictated by the needs of the patient.
  • Identifiesresources for clients to overcome barriers to care, such as transportation,housing,and health insurance coverage.
  • Explains the Marketplace application process to our clients, program requirements and fully informs them of theiroptions.
  • Explains Individual and Small Business Health Packages touninsured patients and provides them with the opportunity toparticipatein the program.
  • Ensures all Individual and Small Business Health Package payments are chargedin accordance withsigned contractsand that allmembers are scheduled foran initialmedical wellness exam.
  • Sends out monthly Customer SatisfactionSurveys;Reviewsfeedback andidentifiesareas ofopportunity for changes and improvements of patient care, including medical, behavioral, family planning, spa, and any other area of 26Health patient focus.

  • Collaborates with Training & Education team to develop and suggest enhancements to training plans that further support patient experience excellence.

Qualifications/Knowledge/Skills/Experience
  • High school diploma or equivalent.
  • Bilingual (English/Spanish) proficiency isrequired.
  • 2+ years in a related role within a healthcare organization is preferred.
  • Must be willing and able to complete a required 20+ hour online training course and pass final exam for Federal certification prior to assuming Certified Application Counselor duties.
  • Must have the ability to work flexible hours, including evenings andweekends,as needed.
  • Excellent verbal and written communication skills; communication style that is open and fosters trust, credibilityand understanding.
  • Proficient in all MS Office applications (Excel, Word, PowerPoint)
  • Experience workingwithECWor other EHR system highly preferable.
Working conditions
  • Prolonged periods standing, walking, and/orsitting at a desk and working on a computermay berequired. Work may berequiredin various environments, both inside and outside of the office,as needed by project/activity.
