About The Role

The Patient Navigator plays a key role in guiding patients through their healthcare journey by providing support, education, and coordination of services. This role ensures patients have timely access to care, assists with appointment scheduling, insurance navigation, and addresses barriers to care to enhance patient experience and improve outcomes.

Responsibilities
  • Serve as the primary point of contact for patients requiring assistance navigating the healthcare system.
  • Coordinate patient appointments, follow‑ups, and referrals to ensure timely care.
  • Assist patients with understanding treatment plans, medical instructions, and available resources.
  • Identify and address potential barriers to care such as transportation, financial concerns, or language barriers.
  • Provide education on insurance coverage, copays, and authorizations, working with billing teams as needed.
  • Collaborate with providers, nurses, case managers, and other staff to ensure coordinated patient care.
  • Track patient progress and follow up on missed appointments, ensuring continuity of care.
  • Document all patient interactions, follow‑ups, and outcomes accurately in the Electronic Health Record (EHR).
  • Support quality initiatives by monitoring patient satisfaction and identifying areas for service improvement.
  • Maintain confidentiality and comply with HIPAA and company policies.
Requirements
  • High School Diploma or GED required; Associate’s or Bachelor’s Degree in Health Sciences, Social Work, or related field preferred.
  • 2+ years in Patient Navigator, Care Coordinator, Case Manager, or Discharge Planning roles.
  • Experience in behavioral health settings (CSU, inpatient psych, rehab, PHP/IOP).
  • Familiarity with EHR systems and healthcare workflows.
  • Strong understanding of insurance processes and medical terminology.
  • Excellent communication, problem‑solving, and organizational skills.
  • Compassionate, patient‑centered approach with the ability to work with diverse populations.
  • Experience in behavioral health or specialty outpatient services.
  • Knowledge of community resources and support programs.
  • Ability to manage multiple tasks in a fast‑paced environment.

Additional Information

The expected base pay for this role will be between $40,000 and $50,000 annually at the commencement of employment. Base pay will be determined on an individualized basis and will be impacted by location and years of experience. Base pay is only part of the total compensation package, which may also include incentive compensation, discretionary bonuses, other short‑ and long‑term incentive packages, and other Psychplus health‑sponsored benefits.
