Community Healthcare Network is seeking a Patient Navigator for Health Homes Care Coordination in New York, NY. This full-time role supports the Care Management team by locating patients, maintaining rosters, linking patients to care, escorting appointments, and educating patients to promote retention in primary care.

Qualifications

Responsibilities

• Assist the Care Manager in providing services for the full caseload.
• Locate patients assigned to the team and update roster information.
• Link patients to care and educate them on health conditions.
• Provide appointment reminders and escort to appointments.
• Maintain case records and scan documents into the EMR.
• Assist with office administration and delivery of direct services.
