Job summary

Cabarrus Rowan Community Health Centers seeks a Patient Navigator to support the QI Director in planning and implementing a systemic navigation program. The role focuses on eliminating barriers to timely care, improving patient retention, and supporting quality outcomes across the healthcare continuum.

Responsibilities include coordinating appointment scheduling, patient education, and connections to support services, while assisting senior leadership with quality and performance improvement

Responsibilities

• Acts as liaison between patient, family and service providers; facilitates appointments and referrals.
• Assesses patient understanding and provides education/resources.
• Navigates the system for scheduling, forms, demographics, and referrals.
• Empowers patients to self-navigate through education and marketing initiatives.
• Supports Care Coordination with outreach for preventive screenings and education.
• Provides health information resources and advocates multidisciplinary care.
• Promotes navigation program within CRCHC community and staff.
• Maintains resource database for client support.
• Identifies barriers to care and connects to resources.
• Monitors grant/contract compliance with CFO and Administrative Director.
• Collects and analyzes patient feedback; supports quality reporting.
