Qualifications

• Ability to read, write and comprehend instructions in English.
• Proficient in Microsoft Office products; strong written and verbal communication.
• Experience sought: 1–2 years in related work or customer service.
• Knowledge of medical terminology and office protocols is preferred.

Responsibilities

• Acts as liaison between patient, family and service providers; facilitates scheduling and referrals.
• Assesses patient understanding and provides education and resources to address needs.
• Assist patients in navigating the system: schedule next appointments, complete forms, update demographics, coordinate referrals.
• Educate and empower patients to self-navigate the healthcare system; support marketing initiatives.
• Support Care Coordination with outreach for preventive screenings and education.
• Provide guidance to staff on quality improvement projects related to patient retention, satisfaction, or recovery.
