Job summary

ChristianaCare is seeking a Patient Navigator to guide patients through defined health care pathways, coordinate appointments, and remove barriers to timely care. You will review referrals, orient patients and families, and ensure smooth transitions to ongoing services.

The role emphasizes collaboration with clinical and operational teams, tracking milestones, handling scheduling and documentation in the electronic health record, and maintaining HIPAA compliance.

Qualifications

• High school diploma or GED required.
• Associate or bachelor’s degree in health-related field preferred.
• Experience in patient navigation or care coordination preferred.

Responsibilities

• Receive and review referrals for assigned clinical pathways and confirm patients meet criteria.
• Orient patients and families to the pathway, including purpose, steps, responsibilities, and contacts.
• Guide patients through required appointments, tests, referrals, and milestones using pathway workflows.
• Coordinate pathway scheduling, reminders, instructions, records, forms, and handoffs with clinical teams.
• Track patient status against milestones and follow up on missed or incomplete steps.
• Identify barriers (transportation, language, scheduling, documentation) and connect to resources.
• Communicate pathway status and barriers to the clinical/operational teams per escalation protocols.
• Support closed-loop referrals by confirming receipt and disposition of referrals.
