Zusammenfassung

ChristianaCare is seeking a Patient Navigator to guide patients through defined healthcare pathways, coordinate appointments, and resolve barriers to timely care. You will work with clinicians and care teams to ensure seamless transitions and accurate documentation in the electronic health record.

Required is a high school diploma or GED, with strong communication skills, attention to detail, and commitment to patient-centered care.

Aufgaben

• Receive and review referrals for assigned clinical or program pathways and confirm that patients meet established navigation criteria.
• Orient patients and families to the pathway, including purpose, steps, responsibilities, and points of contact.
• Guide patients through the required sequence of appointments, tests, procedures, referrals, services, or milestones.
• Coordinate pathway-specific scheduling, reminders, preparation instructions, records, forms, and handoffs with clinical and operational teams.
• Track patient status against milestones and follow up on missed, delayed, or incomplete steps.
• Identify barriers such as transportation or language access and connect patients to resources.
• Communicate pathway status and barriers to clinical, operational, social care, or program teams per escalation protocols.
• Support closed-loop referrals by confirming receipt, scheduling, completion, or disposition.
• Provide approved information about the pathway and reinforce clinician-established instructions.
• Facilitate transitions to the patient’s usual care team or community resources when indicated.
• Close navigation services when criteria are met and document completion.
• Document outreach, milestones, barriers, referrals, escalations, and closure accurately in the EHR or program system.
• Participate in pathway huddles, interdisciplinary meetings, training, and quality improvement activities.
• Identify recurring process barriers and report trends to improve pathway access, timeliness, coordination, and patient experience.
• Maintain confidentiality and comply with HIPAA and organizational policies.
• Perform work safely and report unsafe conditions to the supervisor.
• Perform other related duties as assigned.
