Job summary

ADP, Inc. is seeking a Community Health Worker/Patient Navigator for the Transitions of Care Team in Worcester, MA. This onsite position supports outreach, education, and referrals to help patients engage in care and navigation services.

The CHW will document services, assist with care plans, and coordinate with internal and external partners to ensure access to resources. Requires two years of relevant experience and bilingual abilities.

Qualifications

• High school diploma or GED required
• Bachelor’s degree preferred
• Community Health Worker or Medical Interpreter certificate of completion preferred
• Minimum of two years relevant work or college-level internship experience

Responsibilities

• Supports the care team with patient follow-up, reminders, and addresses barriers to care.
• Documents services as required and assists in meeting program requirements.
• Collects and analyzes data related to the project and works with data management staff for data collection.
• Provides outreach and meets with patients to explain services and coordinates the treatment plan.
• Facilitates referral process for patients using Healthify, E-referral, Community Help, and other tools as applicable.
• Advocates for patients regarding transportation, interpretation, health insurance, and FHCW services.
• Strong organizational and communication skills to act as a hub of information across multiple departments.
• Any and all other duties as assigned.
