Job summary

Ascension Saint Thomas Midtown in Nashville, TN is seeking a Patient Navigator in the Cancer Registry department for a full-time daytime role. You will assess patient needs, guide them to health services, and coordinate appointments with care teams to ensure smooth patient journeys.

Qualifications

• BLS Provider certification preferred.
• Driver's license preferred.
• Experience in patient navigation or health services is a plus.

Responsibilities

• Provides informal counseling, education, and social support; assists with scheduling medical appointments and applications.
• Builds strong relationships with external referral sources and internal teams.
• Maintains accurate client records documenting contacts, services, and outcomes.
