Job summary

Qualifications

Responsibilities

• Identifies potential clients, completes initial needs assessment, and follows up to provide guidance to health services.
• Provides informal counseling, education, and social support, assists with scheduling medical appointments and accompanying clients.
• Builds and maintains relationships with external referral sources and internal teams.
• Maintains accurate client records by documenting contacts, services and outcomes for future reference.
