Job summary

Ascension Saint Thomas Midtown in Nashville, TN is seeking a Patient Navigator in the Cancer Registry department for a full-time daytime role. You will assess patient needs, guide them to health services, and coordinate appointments with care teams to ensure smooth patient journeys.

You will build relationships with external referral sources and internal teams, document contacts and outcomes, and help patients access available resources.

Qualifications

• BLS Provider certification preferred.
• Driver's license preferred.
• Experience in patient navigation or health services is a plus.

How you'll make an impact in this role

• Identifies potential clients, completes initial needs assessment, and maintains follow-up to ensure appropriate guidance to health services.
• Provides informal counseling, education, and social support to clients. Assists clients with utilizing available resources, including scheduling medical appointments, accompanying clients to appointments, and assisting with completion of medical program applications.
• Develops and maintains strong working relationships with external (referral sources and community agencies) and internal contacts (departmental team members).
• Maintains accurate client records by documenting all contacts, services provided, and outcomes in order to track all clients and services for future reference.

What minimum requirements you'll need

Licensure / Certification / Registration

• Preferred Credential(s):
• BLS Provider. American Heart Association or American Red Cross accepted.
• Drivers License.
