Job summary

Community Healthcare Network is seeking a Patient Navigator to join the Care Management team at multiple sites across New York, NY. The role supports Care Managers by locating patients, updating rosters, linking patients to care, escorting appointments, and maintaining case records in the EMR.

Strong organizational skills and attention to detail are essential. Requirements include a High School Diploma or GED and at least two years of office experience; bilingual skills are a plus.

Qualifications

• High School Diploma or GED required.
• Two years office and/or related experience preferred.

Responsibilities

• Assist the Care Manager in provision of services for the entire caseload.
• Locate patients assigned to the team and update roster information.
• Link patients to care and conduct appointments’ escort.
• Educate patients on health conditions and provide appointment reminders.
• Maintain the case record and scan documents into the EMR.
• Provide direct concrete services to patients to ensure retention in primary care.
