Job summary

Crossroads Health is seeking a compassionate Patient Navigator-Behavioral Health to be the first contact for clients and families seeking behavioral health and primary integrated services. The role focuses on trauma-informed screening, intake documentation, and coordinating care in collaboration with a multidisciplinary team.

Ideal candidates hold an Associate’s degree in human services with a preferred Bachelor’s in social work, Ohio licensure (LPC/LSW/MFT) preferred, and QMHS eligibility

Qualifications

• Licensed in the State of Ohio with LPC/LSW/MFT (preferred).
• QMHS certification to be acquired within 15 days of hire.
• Prior experience in a medical office setting with substantial patient interaction (preferred).

Responsibilities

• Serve as first contact for new and returning clients by phone and in person.
• Screen incoming clients to identify service needs and coordinate care.
• Provide trauma-informed screening, triage, and care coordination for clients and families.
• Facilitate communication among treatment providers and clients to ensure engagement and access to care.

We are currently searching for a compassionate person to join our Crossroads Health team in the role of Patient Navigator-Behavioral Health. The Patient Navigator is often the first point of contact for new and returning clients. This position is responsible for responding to and screening incoming clients to identify service needs. This provider will engage as a member of a collaborative team. With a Trauma Informed approach, this position will be a representative for the organization and how we offer integrated services for recovery and mental health.

Most importantly, the position will:

• Provide trauma informed services to clients primarily on the phone and in the office.
• Assist clients in completing initial required documentation.
• Providing trauma informed individual screening, triage and care coordination to clients and their family system in an office setting as they present for walk-in or call into the intake line.
• Assisting in communication amongst treatment providers and with clients to ensure strong engagement and access to care.

Position Requirement:

• Associate’s Degree in Human Services or related field required. Bachelor’s degree in social work or related field (preferred).
• Licensed in the State of Ohio with LPC/LSW/MFT (preferred)
• Candidates must be willing to acquire QMHS within 15 days of hire.
• Prior experience in medical office setting, with substantial patient/client interaction (preferred).
