Pinnacle Fertility in Brooklyn, NY is seeking a Patient Navigator to join our on-site team. You will guide patients through their treatment plans, coordinate appointments, and help families understand care options and insurance resources.

This full-time role requires 2+ years in clinical healthcare, strong communication, EMR proficiency, and a compassionate approach. Travel between New York offices may be required to support patient care.

Qualifications

• 2+ years in a clinical healthcare setting focused on patient care coordination, navigation, or case management.
• Experience using EMR and healthcare software.
• Bilingual skills are a plus.

Responsibilities

• Serve as primary point of contact for patients, guiding them through treatment plans and educating on procedures and follow-up care.
• Coordinate and schedule appointments, tests, and referrals for a smooth continuum of care.
• Assist patients with understanding medical care, insurance coverage, and financial resources.
• Communicate with healthcare providers, clinical staff, and administrative teams to ensure seamless care coordination.
• Maintain accurate patient records and care plans in the EMR system.
• Provide emotional support and resources to patients and families.
• Participate in team meetings and quality improvement initiatives to enhance care delivery.
• Educate patients on treatment protocols and self-care strategies.
• Track patient progress and follow up on missed appointments.
• Advocate for patients’ needs and resolve barriers to care.
• Assist with after-hours call coverage on a rotating schedule.

About the Role

The Patient Navigator position at Pinnacle will allow you to engage with the patient throughout their journey while working closely with a multidisciplinary team to provide excellent patient care.

We are seeking a Patient Navigator to join our dedicated team at Pinnacle Fertility - New York located in Brooklyn, NY. This is a full-time, on-site position. The role may require travel and floating between our New York office locations as needed to support operational and patient care needs.

Key Responsibilities

• Serve as the primary point of contact for patients, guiding them through their treatment plan and providing education on procedures, testing, and follow-up care.
• Coordinate and schedule patient appointments, tests, and referrals to ensure a smooth continuum of care.
• Assist patients with understanding their medical care, insurance coverage, and available financial resources.
• Communicate with healthcare providers, clinical staff, and administrative teams to ensure seamless patient care coordination.
• Maintain accurate and up-to-date patient records, including documentation of interactions and care plans in the electronic medical record (EMR) system.
• Provide emotional support and resources to patients and their families, addressing any concerns or barriers to care.
• Participate in team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery.
• Educate patients on treatment protocols, self-care strategies, and medication usage.
• Track patient progress, follow up on missed appointments, and ensure compliance with treatment plans.
• Advocate for patients’ needs and help resolve any issues or barriers to care.
• Assist with after-hours call coverage on a rotating schedule, ensuring patient needs are addressed appropriately.
• Other duties and project as assigned

Position Requirements

• Education: Highschool Diploma required. Bachelor’s degree (preferred)
• Experience: Minimum of 2 years of experience in a clinical healthcare setting, including patient care coordination, patient navigation, or case management.
• Skills:
  • Strong knowledge of healthcare systems, insurance processes, and patient advocacy.
  • Excellent communication, organizational, and problem-solving skills.
  • Proficiency in using electronic medical records (EMR) and other healthcare software.
  • Bilingual skills are a plus.

COMPENSATION & BENEFITS

• Hourly Rate: $24.00 - $30.00 per hour (Final offers based on experience, skills, and qualifications).
