We Are Hiring
  • Department: Community Health Access
  • Schedule: Monday-Friday, 8am-4:30pm, unless otherwise indicated for outreach events
  • Hospital: St. Vincent Evansville
  • Location: Evansville, IN
What You Will Do

Coordinate access to health-related services by assisting clients with utilizing available resources.

Responsibilities:

  • Identify potential clients, complete initial needs assessments, and follow up to ensure appropriate guidance to health services.
  • Provide informal counseling, education, and social support. Assist clients with scheduling medical appointments, accompanying them to appointments, and completing medical program applications.
  • Develop and maintain strong working relationships with external referral sources, community agencies, and internal team members.
  • Maintain accurate client records by documenting contacts, services provided, and outcomes for future reference.
What You Will Need

Education:

  • High School diploma or equivalency with 2 years of experience, or an Associate's/Bachelor's degree, or 4 years of applicable job-specific experience.
Additional Preferences

Customer service experience, familiarity with community resources and insurance.
