Overview

Care Navigator

Looking for a way to make a difference in people’s lives?

As a Care Navigator, you will play a critical role in supporting our members to live the best, rest of their lives. This position acts as the primary point of contact for our members, their families, and our partner senior living facilities. You will help members navigate through the healthcare landscape and advocate for their highest quality of care.

The position is remote and requires visits to facilities regularly. Duties include, but are not limited to:

Member Engagement:
  • Main contact for Members and families upon Plan enrollment
  • Conduct welcome interviews and Health Risk Assessments, with routine follow-ups
  • Assist members with referrals, appointment scheduling, DME orders, and accessing Plan benefits
  • Participate in Care Planning meetings as appropriate
  • Coordinate care among specialists, home health, therapy, Plan APP, PCP, etc.
Facility Engagement:
  • Act as the liaison for facilities, connecting them to Plan resources like APP, sales, and leadership
  • Assist with customer service requests such as billing, authorizations, or member transfers
Clinical Support:
  • Collect medical records from community sources (H&P’s, medication history, specialist notes, etc.)
  • Support Utilization Management and APPs with record collection
  • Monitor care transitions and notify the Plan APP and utilization management
  • Monitor enrollment status changes and notify Plan APP

Qualifications
  • At least 1 year experience in healthcare, social services, or patient engagement
  • Experience in senior living facilities is strongly preferred
  • Computer literate, proficient with email, internet, and databases
  • Excellent verbal and written communication skills; able to produce reports
  • Pleasant personality and team-oriented
