• Provide follow-up in-home visits and escalate priority issues as needed.

The Patient Navigator shall be highly visible customer serve role that provides a friendly experience when escorting patients, family members, and guest to their designated destination upon arrival and after registration.---***To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
