Alivio Medical Center Inc. seeks a Patient Navigator to create a welcoming environment for patients and visitors at AMC and Cook County Health & Hospital Systems. You will escort patients, assist at check-in, and provide information about services.

The role demands excellent customer service, bilingual communication, and the ability to work with diverse populations. Responsibilities include directing patients to destinations, responding to inquiries, and performing follow-up visits as needed.

Qualificações

• Bilingual Spanish/English required.
• Strong customer service skills essential.
• Ability to stand and walk for prolonged periods during shifts.
• Ability to follow detailed instructions and maintain accuracy.
• Must be able to maintain confidential patient information per HIPAA rules.

Responsabilidades

• Greet and assist patients and guests in a courteous manner.
• Escort patients to designated destinations and provide directions.
• Answer inquiries and provide information about AMC and CCHHS services.
• Assist with check-in at kiosks and follow up with in-home visits as needed.

Summary:The Patient Navigator will create a welcoming environment for patients and guests of Alivio Medical Center (AMC) and Cook County Health & Hospital Systems (CCHHS). The Patient Navigator shall be highly visible customer serve role that provides a friendly experience when escorting patients, family members, and guest to their designated destination upon arrival and after registration.To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
