Qualifications

• Bachelor’s degree preferred.
• HIPAA training required upon hire.
• 5–7 years of healthcare/patient services experience.
• Experience with Epic, Outlook, Word, Excel, PowerPoint; familiarity with philanthropy CRM.

Responsibilities

• Serve as a primary resource for patients via phone, email, and in-person interactions.
• Coordinate and expedite appointments, referrals, transfers, and access to specialty care across BILH and community hospitals.
• Collaborate with leadership to support navigation needs across sites.
• Support patients and families in understanding care pathways and resources.
• Maintain documentation and manage patient navigation databases.
