Job summary

Heritage Hall Healthcare & Rehab Center is seeking a Patient Liaison/Scheduler to help ensure residents receive high-quality care and to support the Admissions Director and nursing management. This role focuses on internal family resident liaison, efficient transport scheduling, and prompt communication to maintain exceptional service standards.

Responsibilities

• Serve as an internal liaison and customer service resource for families, residents, and visitors.
• Schedule resident transports when needed.
• Respond promptly to calls and support Admissions Director and nursing management.
