Job summary

Jobtailor is seeking a healthcare liaison to coordinate home infusion services across hospital discharge planners, case managers, prescribers, and the infusion team.

You will meet patients to assess therapy suitability, explain insurance options, and educate on medication administration and equipment, while collaborating with nurses and pharmacists to ensure safe transitions home.

Qualifications

• High school diploma or GED required.
• Certification as Medical Assistant or Pharmacy Technician preferred.
• 1–3 years of healthcare or related experience preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Serve as the primary contact among discharge planners, case managers, prescribers, and the home infusion pharmacy.
• Meet patients referred for home infusion services to determine therapy appropriateness.
• Review insurance coverage, out-of-pocket costs, and payment options.
• Educate patients and caregivers on safe medication administration and infusion equipment.
• Collaborate with nurses and pharmacists to review clinical documentation for payer approval and safe transition.
• Identify treatment-plan barriers and work with the care team to resolve them.
• Facilitate communication between hospitals, providers, and the home infusion pharmacy team.
• Participate in quality improvement activities related to patient transitions and referral satisfaction.
• Build clinical relationships across system hospitals.
• Introduce home infusion services and ensure patients understand medication self-administration.
• Collaborate with hospital case managers and discharge planners to facilitate transitions home.

Requirements

• High School diploma or GED required
• Certification or experience as a Medical Assistant or Pharmacy Technician preferred
• 1–3 years of work experience in healthcare services, specialty pharmacy, patient services, customer service, or medical office support preferred
• Strong communication and interpersonal skills
• Ability to interact effectively with patients, caregivers, case managers, and providers
• Organizational skills with ability to manage multiple referrals, track authorizations, and coordinate with clinical teams
• Ability to travel between referral sources
• Ability to prioritize tasks, manage multiple referrals, and meet deadlines
• Strong presentation skills for in-services and referral source education
• Sound judgment in escalating issues to nurses, pharmacists, or leadership when appropriate
• Ability to work collaboratively with clinical staff and referral partners
• Demonstrated commitment to valuing diversity and contributing to an inclusive working and learning environment
• Ability to perform the stated physical requirements
