Job summary

Jobtailor is seeking an experienced Infusion Nurse for home health infusion services in Connecticut. The role involves direct patient care in homes or infusion suites, developing nursing plans, and educating patients and families to support therapy independence.

The position requires an active RN license, CPR certification, and 1+ year infusion nursing experience, with travel up to 100%. BSN preferred; strong documentation and teamwork essential.

Qualifications

  • Active and unrestricted RN license in the state of practice.
  • Minimum of 1 year nursing experience
  • Current CPR certification required
  • Demonstrated competency in patient care standards required for safe delivery of services and applicable infusion skill sets
  • Advanced certification and training as applicable
  • Required licensure to operate a motor vehicle in the state of practice
  • Access to a vehicle for business travel with proof of liability insurance
  • Bachelor of Science in Nursing from an accredited school of nursing preferred
  • 1–2 years of previous infusion nursing experience preferred

Responsibilities

  • Coordinate and provide intermediate direct patient care in patients’ homes or alternate infusion suites
  • Initiate, develop, implement, and evaluate intermediate nursing plans of care
  • Perform treatments, administer medications, and educate patients and families/caregivers
  • Modify treatment plans based on patient status and physician orders
  • Assess patient needs and physical status during skilled visits
  • Obtain and clarify physician orders and promptly report significant patient‑condition changes
  • Provide training and mentorship to Infusion Nurse I’s
  • Teach patients and families using a patient‑centered approach to support therapy independence
  • Communicate with clinical teams, medical providers, patients, and caregivers to ensure continuity of care
  • Prepare real‑time clinical documentation and submit nursing records for billing timelines
  • Coordinate discharge planning and prepare discharge summaries
  • Comply with agency policies, professional standards, regulations, and accreditation requirements
  • Use computers and automation technologies to validate treatment orders and document care
  • Respond to patient requests and initiate emergency procedures as needed
  • Safeguard patient and proprietary information in accordance with HIPAA
  • Explain Option Care services and related financial obligations to patients
  • Complete required training, in‑services, and continuing education
  • Provide oversight and coordination of paraprofessionals and act as supervisor when applicable
  • Participate in multidisciplinary conferences, precepting, training, mentoring, and nursing on‑call responsibilities
  • Insert PICC/Midlines, troubleshoot or declot central lines, and educate on line care when applicable

Job description

Requirements

  • Bachelor of Science in Nursing from an accredited school of nursing preferred
  • 1–2 years of previous infusion nursing experience preferred
