Job summary

Jobtailor seeks a healthcare sales liaison (RN or Pharmacist) in New York to maintain and grow referral-source relationships, educate providers, and support discharge planning across partner facilities, hospitals, and medical practices. You will travel to care sites to process referrals, provide education, and help implement treatment plans that enable timely discharge and home or alternate-site care.

This role requires strong relationship-building, customer service, and knowledge of ACHC/URAC

Qualifications

Job description

  • Maintain and grow referral-source relationships to increase sales and patient starts
  • Serve as a key point of contact and representative of Option Care, providing education, assistance, and service to referral sources
  • Interact with area service providers daily to sell Option Care services for discharging patients
  • Assess patients selected by hospitals to determine suitability for alternative-site care
  • Educate patients, families, facility staff, and caregivers about Option Care services and products
  • Provide hands-on, in-person education to patients and caregivers to support timely discharge and therapy independence
  • Communicate with agency staff, medical teams, patients, and families throughout discharge to implement treatment plans
  • Collect, review, complete, and secure signoff for pre-admission information
  • Partner with the Account Manager and Regional Sales Director on area business and growth plans
  • Coordinate with Account Manager and marketing staff on educational and promotional programs
  • Review medical records for relevant history and insurance information and communicate it to patient registration
  • Coordinate transitions with referral sources and internal partners to support seamless transitions to home or ATS
  • Facilitate transfer of orders by phone, fax, and e-prescribing
  • Maintain confidentiality of patient and proprietary information
  • Provide oversight and input regarding patient assessment, treatment, and transition to home care
  • Coordinate communication with other providers
  • Arrange special medical supplies or appliances
  • Travel to partner facilities, community hospitals, and medical practice offices to sell services, process referrals, and provide education and training
  • No supervisory responsibilities
Requirements
  • Licensed Registered Nurse (RN) or licensed Pharmacist in the state of practice
  • At least 2 years of experience in the healthcare industry
  • Experience establishing and maintaining relationships with individuals at all levels of the organization, in the business community, and with vendors
  • Experience applying knowledge of standard practices for all services offered and current relevant and applicable standards, including ACHC and URAC standards
  • Experience providing customer service to internal and external customers, including meeting quality standards and evaluating customer satisfaction
  • Basic PC skills, including internet, email, Microsoft Word, Microsoft Excel, and Microsoft PowerPoint
  • Experience identifying operational issues and recommending and implementing strategies to resolve and improve processes
  • Reliable means of transportation for travel to care facilities, home visits, and multiple hospitals
  • Valid driver’s license and proof of insurance if using a personal vehicle
  • Ability to plan, organize, and make presentations
  • Hands-on home or alternate site infusion or discharge planning experience preferred
  • Experience growing service provider partnerships preferred
