Job summary

Qualifications

• Active and unrestricted RN license.
• Minimum of 2 years of experience in the healthcare industry.
• Experience establishing and maintaining relationships at all levels of organizations and with vendors.
• Familiarity with ACHC and URAC standards and current applicable guidelines.
• Proven customer service experience with internal and external customers and meeting quality standards.

Job description

• Evaluate, educate, and train patients, caregivers, and facility staff on how Option Care Health services and products are facilitated in alternative-site, in-home, or virtual settings
• Conduct patient assessments and evaluations to determine viability for the Option Care Health care delivery model
• Partner and coordinate with the sales team to understand clinical transition education needs and improve patient transition volume
• Maintain confidentiality of patient and proprietary information and follow legal safeguards
• Document education using Option Care Health applications and technology
• Coordinate transitions from hospital to home or alternate care settings with facility staff, providers, and internal teams
• Support timely discharge, therapy education, and continuity of care
• Provide in-person patient and caregiver education
• Gather pre-admission and insurance information
• Facilitate initiation of services for safe and effective care at home
• Travel locally to partnership facilities, community hospitals, and medical practice offices to sell Option Care services, process referrals, and provide live education and training support

Requirements

• Active and unrestricted Registered Nurse (RN) license required
• Minimum of 2 years of experience in the healthcare industry
• Experience establishing and maintaining relationships with individuals at all levels of the organization, in the business community, and with vendors
• Experience applying knowledge of standard practices for all services offered and current relevant and applicable standards, including ACHC and URAC standards
• Experience providing customer service to internal and external customers, including meeting quality standards and evaluating customer satisfaction
• Basic PC skills, including internet, email, Microsoft Word, Microsoft Excel, and Microsoft PowerPoint
• Experience identifying operational issues and recommending and implementing strategies to resolve issues and improve processes
• Reliable means of transportation for travel to care facilities, home visits, and multiple hospitals
• If using a personal vehicle, a valid driver’s license and proof of insurance are required
• Willingness to obtain nursing licensure in additional states if business need and geography support it
• Ability to plan, organize, and make presentations
• Preferred: hands-on home or alternate site infusion or discharge planning experience
• Preferred: experience growing service provider partnerships
