Job summary

Greene Point Health & Rehab is seeking a Social Services Leader to plan, direct, and coordinate the social services program within a mission-driven health system. You will organize activities that promote social interaction and cognitive engagement for residents, while coordinating admissions and working with families and community agencies.

The role requires counseling skills, documentation proficiency, and the ability to collaborate with care teams to support residents’ emotional well-being

Qualifications

• Bachelor degree preferred or minimum two years’ supervisory hospital/medical center experience.

Responsibilities

• Plan, develop, organize and direct the Social Services Program.
• Coordinate family and patient activities for social interaction and stimulation.
• Counsel patients and families including death-related feelings and other limitations.
• Complete social service portion of the MDS within 14 days of admission.
• Coordinate admissions and discharges.
• Act as liaison with social, health, and community agencies.
• Facilitate Patient and Family Councils as appropriate.
• Assist with grievance resolution and advance directives documentation.
• Promote the System’s image through servant leadership and open communication.
• Contribute to committees and project work; perform other duties as needed.
