Job summary

Qualifications

• Master's degree in gerontology or related field preferred.
• At least one year of experience in a social services program for the elderly.
• Ability to assess psychosocial needs and develop goals.
• Ability to plan, implement with minimal supervision.
• Knowledge of DSM and resident care planning.

Responsibilities

• Assess residents' psychosocial needs and develop goals for services.
• Attend care planning conferences and integrate social work goals.
• Coordinate with community resources and discharge planning.
• Supervise social work interns and contribute to staff education.
• Advise on admission processes and maintain confidential records.
• Advocate for resident rights in clinical IDT meetings.
