Job summary

CenterWell Senior Primary Care in Orlando, FL is seeking a Patient Service Advisor to engage patients in their care, educate them on services, and guide them through transitions. You will support onboarding, enhance patient experience, and drive growth and retention metrics across the center, reporting to the Associate Operations Director.

Qualifications

• 2+ years of experience in a healthcare setting or related business environment.
• Proficient with Microsoft Office (Word, Excel, PowerPoint).
• Excellent customer service with strong verbal and written communication.
• Detail-oriented with solid organizational and time-management skills.
• Comfortable discussing sensitive topics and supporting patients through transitions.
• Collaborative team player with strong interpersonal skills.

Responsibilities

• Oversee the implementation of growth and retention initiatives within the center.
• Develop familiarity with patients to personalize their care and experience.
• Assist new patients with registration and orientation to the model of care.
• Educate patients about services, benefits, and events available.
• Conduct 30/60/90 day follow-up outreach after first appointment.
• Support onboarding and retention through in-clinic and telephonic outreach.
• Coordinate with staff to complete forms, screeners, and handoffs.
• Execute lead nurture journeys to convert prospects to active patients.
• Capture patient feedback and escalate issues with cross-functional teams.
• Document patient interactions in EMR and CRM systems.
• Partner with brokers and community engagement to raise awareness.
• Stay informed about local trends to advocate for center strategies.
• Monitor growth metrics and use dashboards to inform improvements.
