Qualifications

• High school diploma or GED required.
• Ability to work in a multicultural community-based organization.
• Commitment to confidentiality and professional boundaries.
• Experience with substance abuse treatment preferred.
• Clear UDS and criminal history required.

Responsibilities

• Complete EMR notes accurately and on time.
• Monitor group participation and attendance.
• Orient patients to the program and handbook.
• Provide educational and support groups.
• Assist with transport for PHP patients to off-site activities.
• Support detox patients daily and assist engagement.
• Assist with AMA discharge rescues.
• Be available for crisis intervention.
• Use coping skills with patients.
• Plan and execute monthly engagement activities.
• Document weekly engagement and retention outcomes.
• Coordinate engagement activities with defined skill sets.
