Job summary

Galen Medical Group is seeking a Patient Engagement Coordinator in the Chattanooga region. This role focuses on promoting patient engagement and coordinating outreach to improve care metrics, referrals, and preventive services under the Population Health program.

The PEC will manage patient rosters, track outreach in the EHR, and connect patients with PCPs, specialists, and insurance-related care management programs while ensuring confidentiality and compliance with HIPAA.

Qualifications

• Excellent customer service skills.
• Exceptional verbal and written communication, problem solving, and professional interaction.
• Self-reliant with the ability to work independently and meet goals.
• Detail oriented and well organized.
• PC literacy with Microsoft Office (Excel, Word, etc.).
• Strong time management and ability to meet deadlines.
• Knowledge of basic medical terminology is helpful.

Responsibilities

• Outreach to encourage engagement with PCPs and specialists.
• Maintain patient rosters and track outreach in the EHR.
• Provide telephonic outreach under Value-Based Care programs.
• Ensure scheduled services are completed.
• Maintain professional rapport with clinicians and staff.
• Participate in team and department meetings.
• Uphold HIPAA and confidentiality.
• Support mission and values.
