Job summary

Better Health Group is seeking a Health Coach in Florida to engage patients by phone, coordinate care after hospital events, and promote preventive services. You will support medication adherence, address Social Determinants of Health, and educate patients on available benefits and resources.

Responsibilities include proactive outreach, coordinating with clinicians, documenting activity in EMR, and ensuring patient confidentiality under HIPAA.

Qualifications

• High school diploma or equivalent required; associate or bachelor preferred.
• 1+ year in healthcare, patient outreach, care coordination, health coaching, or related field preferred.
• Familiarity with Transitional Care Management (TCM), preventive care, medication adherence, and SDOH preferred.
• Strong interpersonal and patient engagement skills; ability to empathize and communicate with diverse populations.
• Excellent verbal and written communication; adept at telephonic outreach and coordination with patients and providers.
• Strong organizational and time-management skills; ability to manage multiple outreach activities and maintain documentation.

Responsibilities

• Conduct proactive telephonic outreach to patients to support engagement in recommended healthcare services and follow-up activities.
• Coordinate Transitional Care Management outreach after hospital discharges and other events.
• Support efforts to reduce avoidable readmissions by reinforcing discharge instructions and facilitating follow-up care.
• Schedule Annual Wellness Visits and other preventive appointments per outreach protocols.
• Identify Social Determinants of Health barriers and refer to appropriate resources or teams.
• Educate patients on healthcare benefits and services to promote ongoing engagement.
• Document outreach, coordination, barriers, and follow-up in designated systems.
• Collaborate with clinical and operational teams to address patient concerns and ensure continuity of care.
