Job summary

ChenMed is seeking a Care Facilitator to be a patient-facing first contact at our Saint Petersburg clinic. You will greet and check in/out patients, schedule appointments, verify insurance and collect copays, and assist with referrals and follow-up care.

You will handle phone inquiries, coordinate transportation, and maintain patient confidentiality while delivering exceptional customer service. Evening/weekend availability may be required to support clinic operations.

Qualifications

• High school diploma or equivalent required.
• Graduation from Medical Assistant program preferred.
• Minimum 1 year of work experience in a medical clinic or similar environment.
• BLS for Healthcare Providers preferred.

Responsibilities

• Greet, check-in and check-out patients; schedule appointments and maintain lobby organization.
• Serve as gatekeeper for PCP and specialist schedules and related coordination.
• Enter patient information into EMR, verify insurance and collect copays.
• Make love calls, send birthday cards, and maintain patient communications.
• Answer phones; relay messages and direct to appropriate staff.
• Assist with referrals and follow-up scheduling as needed.
• Escort patients to exam rooms and ensure comfort.
• Support HEDIS-related needs and report hospitalizations when applicable.
• Assist with transportation coordination and ride scheduling as needed.
• Maintain confidentiality of patient information and documents.
