Job summary

Live Well Homecare is seeking a licensed physical therapist to assess, plan, and implement rehabilitative programs to improve mobility, reduce pain, increase strength, and prevent deformities.

You will develop treatment plans with patient and family input, deliver therapy sessions, educate patients and families, and coordinate discharge and follow-up care, documenting evaluations and treatments per protocols. Collaboration with physicians and goal setting ensure quality care across the continuum.

Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy and PT licensure.
• BLS certification and strong communication, customer service, teamwork and computer skills.
• Commitment to ethics, confidentiality, punctuality, professional development, and participation in training and meetings.

Responsibilities

• Develop and revise treatment plans with patient and family input.
• Deliver therapy sessions and educate patients and families.
