LiveWell Homecare Agency is seeking a licensed Physical Therapist to assess, plan, and implement rehabilitative programs across patient care settings. You will work with physicians to develop goals, discharge plans, and optimize mobility while ensuring regulatory compliance.

Responsibilities include patient assessment, care delivery, accurate documentation, and maintaining professional conduct. Strong communication, teamwork, and commitment to ongoing education are essential for success.

Formación

• Graduate from an accredited physical therapy program (Bachelor’s, Master’s, or Doctorate in Physical Therapy).
• Required licenses and certifications include physical therapy licensure and Basic Life Support (BLS).
• Effective communication skills, customer service orientation, teamwork, and basic computer proficiency.

Responsabilidades

• Patient Assessment: Collaborate with physicians to develop and evaluate treatment plans, goals, and discharge strategies based on patient needs.
• Care Delivery: Implement treatment plans, select appropriate activities, and adjust based on patient and family input. Educate patients and provide discharge instructions and referrals.
• Documentation: Record evaluations, treatment sessions, and discharge information following departmental guidelines.
• Professional and Ethical Conduct: Uphold high ethical standards, maintain confidentiality, punctuality, and dress code, and communicate effectively with team members.
• Customer Service & Teamwork: Deliver respectful, positive interactions using AIDET principles, foster teamwork, and support service recovery principles.
• Professional Development: Engage in ongoing education, complete required competencies, and participate in training and orientation activities.
