Job summary

LiveWell Homecare Agency is seeking a dedicated Physical Therapist to assess, plan, and implement rehabilitative programs for patients to improve mobility, reduce pain, and prevent deformities. The role spans care across the continuum with emphasis on quality and regulatory compliance.

The ideal candidate collaborates with physicians on treatment plans, educates patients and families, documents care activities, and ensures ethical, confidential, and efficient care within a homecare setting.

Qualifications

• Graduate from an accredited physical therapy program with a Masters or Doctoral degree.
• Valid physical therapy license and BLS certification are required.
• Effective communication, customer service focus, teamwork, and basic computer skills are essential.

Responsibilities

• Collaborate with physicians to establish and evaluate treatment plans, goals, and discharge strategies.
• Educate patients and families; document care activities; provide discharge instructions and referrals.
• Ensure ethical practice, maintain confidentiality, communicate effectively, participate in staff activities, and utilize resources safely and efficiently.
