Job summary

Qualifications

• Associate or Bachelors’ Degree in Nursing or Nursing Diploma or Master in Social Worker required
• Certification in Case Management preferred
• Certification in InterQual or IQCI preferred
• 2+ years of supervisory experience preferred

Responsibilities

• Promote patient-centered care by coordinating hospital-based case management.
• Oversee day-to-day operations of the case management team and ensure compliance with policies.
• Mentor staff and monitor quality of documentation and assessments to improve outcomes.
