HCA Florida Lake Monroe Hospital is seeking a Director of Case Management to lead daily operations of the CM department and coordinate patient care transitions. The role requires executive presence, strong communication, and the ability to unify interdisciplinary teams and external stakeholders.

You will oversee staffing, resources, and CM processes, collaborating with the CFO and CM leadership to reach established targets while ensuring policy and SOP compliance.

• Executive presence and leadership experience in hospital case management.
• Strong communication and presentation skills.
• Demonstrated change-management capabilities.

Responsibilities

• Directs departmental operations, staffing, and CM model to meet quality goals.
• Allocates resources to staff the department and achieve productivity targets.
• Oversight of CM core functions and clinical practice.
• Ensures care coordination and discharge planning are timely and well documented.
• Partners with CFO and Division CM Leader to achieve goals.
• Monitors performance and drives continuous improvement.
• Ensures compliance with policies and SOP.
• Builds relationships with stakeholders including CMO and CNO.
• Performs other duties as assigned.
• Adheres to Code of Conduct and Mission and Values.
