Job summary

Bon Secours Mercy Health in Toledo, OH seeks a Physical Therapist at St. Vincent Medical Center to provide comprehensive patient care across outpatient, inpatient and pediatrics.

You will perform initial and ongoing assessments, develop treatment plans, and document progress in the EMR, while mentoring PT assistants and collaborating with a multidisciplinary team. Qualifications

• Licensed Physical Therapist in the state of current practice or eligible for trainee/temporary license in state of practice.
• BLS Basic Life Support (preferred on hire, required within 30 days for some sites)

Responsibilities

• Evaluates patients, gathers information, and develops an appropriate treatment plan with goals.
• Provides direct therapy daily according to the plan of care and current standards.
• Maintains accurate records of treatment and patient progress in the EMR; co-signs for assistants when applicable.
• Maintains productivity while ensuring quality care that meets department standards.
• Follows departmental policies on safety, infection control, and timekeeping; completes required education.
• Supervises physical therapy assistants and supports professional growth.
