Job summary

Mercy Health is seeking a dedicated Physical Therapist to perform comprehensive patient assessments and deliver skilled PT interventions at St. Charles Hospital. You will evaluate, treat, and document care across inpatients, outpatients and pediatrics, aligning with each patient’s plan of care.

Responsibilities include maintaining records, teaching family members, and ensuring high-quality care while supervising assistants and meeting productivity standards.

Qualifications

• Evaluates patients and develops goals-based treatment plans.
• Maintains accurate treatment records and progress notes.
• Communicates effectively with patients, families and team.
• Demonstrates age-appropriate care across pediatrics, adults and geriatrics.

Responsibilities

• Evaluates patients and develops treatment plans with goals.
• Provides direct therapy daily per plan of care.
• Maintains and documents patient progress in EMR.
• Co-signs for PT assistants where applicable.
• Supervises PT assistants and upholds safety standards.
