Job summary

Responsibilities

• Create and implement an individualized plan of care with therapeutic interventions.
• Reassess patient progress and adjust treatment as needed.
• Initiate and manage discharge planning from the first visit.
• Document accurately within policy and insurance requirements.
• Collaborate with health care team and communicate with patients and families.
• Provide patient and caregiver education to meet needs.
