Qualifications

• Graduate of an accredited program of occupational therapy.
• Current State Board of Occupational Therapy licensure.
• BLS certification.

Responsibilities

• Performs patient evaluations by establishing an OT profile and goals.
• Establishes an individualized care plan and selects therapeutic interventions.
• Ongoing reassessment of patient progress and response to treatment.
• Initiates discharge planning from first visit and throughout treatment.
• Supervises and delegates to ancillary staff and students per state laws and dept. policy.
• Maintains legible, timely documentation and meets accreditation/insurance requirements.
• Communicates with patient and care team to promote optimal care.

Education

Graduate of an accredited OT program

Current State Board licensure for Occupational Therapy

BLS certification

NBCOT certification (preferred)
