Qualifications

• Graduate of an accredited OT program.
• Current State Board of Occupational Therapy licensure.
• BLS certification.
• NBCOT certification preferred.

Responsibilities

• Performs patient evaluations and establishes goals.
• Develops an individualized plan of care with evidence-based interventions.
• Conducts ongoing reassessment of patient progress.
• Initiates and manages discharge planning from first visit.
• Supervises ancillary staff and students as allowed.
• Maintains accurate documentation within policy timeframes.
• Communicates with patient and care team to optimize outcomes.
• Assesses educational needs of patient and family and provides education.
• Participates in ongoing professional development and quality improvement.
• Ensures safety and compliance with regulations.
• Functions within state practice act.
• Performs other duties as required.
