Job summary

Enhabit Home Health & Hospice is seeking a branch representative to engage external transitional care activities and professional contacts to promote Enhabit Home Health & Hospice services. This role will integrate clinical guidelines to develop patient-centered transition plans that promote quality and efficiency, and educate communities about available care options.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must understand federal, state, and local laws and regulatory guidelines governing hospice operations.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities and professional contacts to promote Enhabit Home Health & Hospice services.
• Integrate clinical guidelines, protocols, and metrics to develop patient-centered transition plans that promote quality and efficiency.
• Serve as a public awareness representative and educate about provided services.
