Qualifications

• Bachelor's degree in nursing or Master's degree in Social Work required.
• Minimum 3 years of clinical experience.
• RN or LMSW license with a valid Michigan license.
• Strong communication, documentation and interpersonal skills.
• Proficiency with computer skills and healthcare systems.

Responsibilities

• Collaborate with the care team to identify and assess patient needs, developing individualized care plans.
• Coordinate services and resources to ensure a safe transition to the next level of care.
• Advocate for patients and families, addressing social determinants of health and connecting them with community resources.
• Evaluate care outcomes and adjust plans to promote patient safety, quality, and cost-effectiveness.
