Job summary

Hackensack Meridian Health in New Jersey is seeking a Patient Care Technician to provide direct patient care and collaborate with nursing staff across shifts. You will help ensure safe, effective outcomes while communicating patient status and participating in care planning from admission to discharge.

Qualifications include a high school diploma or GED and completion of CNA/MA/PCT programs or equivalent medical background; HMH PCT Apprenticeship path available.

Qualifications

• High School diploma or GED.
• Completion of one of: CNA, Medical Assistant or PCT program; or nursing program; or HMH PCT Apprenticeship; or EMT/Home Health Aide/CMA/MA/PCT background.
• Must be able to learn quickly, collaborate with care teams, and provide high-quality patient care.

Responsibilities

• Provide patient care based on age-appropriate criteria with safety and education considerations.
• Ensure safe outcomes through clinical judgment and actions.
• Communicate with patients, nursing staff, and care teams about patient status.
• Take initiative to improve performance and create opportunities.
• Participate in interdisciplinary care planning from admission to discharge.
• Engage in Unit/Department PI activities.
