Job summary

Hospice of Michigan is seeking Care Coordinators to be the front line of the NorthStar Care Community. These individuals must be empathic, professional and knowledgeable in hospice care, as well as in the broader scope of the organization and its partners.

You will communicate with guests, families, and staff; process referrals and admissions; and collaborate with triage RNs to ensure smooth patient care and scheduling.

Qualifications

• Associate degree or 5 years health care/hospice operations experience; bachelor preferred.
• 2+ years telephone-related experience; health care env preferred.
• Strong proficiency in Microsoft Office; Excel/database skills.
• Excellent interpersonal, organizational, confidentiality and communication skills.
• English reading/writing and effective communication for internal/external relations.

Responsibilities

• Communicates courteously with guests, customers, and employees.
• Understands hospice philosophy and services for clear caller communication.
• Processes referrals and schedules admissions as needed.
• Collaborates with triage RNs to ensure efficient patient call flow and dispatch.
• Updates and manages content in patient visit/comfort board and reports.
• Navigate multiple electronic medical records to assist patients and families.
• Enter information into electronic documentation systems promptly.
• Responds to crisis calls and resolves issues for positive outcomes.
• Reviews patient files and reports; handles documents via mail, fax, or email.
• Documents conversations with patients and families in the patient record system.
• Addresses external inquiries with the department operations manager.
• Collaborates with Field Staff Scheduler to coordinate interdisciplinary team staff for admissions.
• Coordinates communication for care provision, including scheduling changes.
