Job summary

Horizon Hospice Care is seeking a Patient Support Specialist to supervise the Intake department and ensure referrals meet state, federal, and company guidelines. You will coordinate with Clinical Leadership and scheduling to maintain smooth admissions and accurate documentation.

The role requires strong communication, organizational skills, and experience in home care settings. Bachelor’s degree is preferred; a high school diploma or GED is required.

Qualifications

• Education requirements include a high school diploma or G.E.D; a Bachelor’s degree is preferred.
• Experience preferred in home care and supervisory roles; strong organizational skills and attention to detail.
• Excellent written and verbal communication skills are essential.

Responsibilities

• Supervise Intake team and ensure timely processing of referrals.
• Coordinate with CARE, Revenue Cycle, Scheduling, Marketing and others to ensure smooth admission.
• Prepare reports and provide data for meetings and leadership.
• Maintain compliance with agency policies and state/federal regulations.
• Assist in interviewing and onboarding staff as needed.
• Promote professionalism and customer service in all interactions.
