Job summary

Pixelcare is seeking a Patient Care Coordinator to support patients through the medication fulfillment process from our Austin headquarters. The role requires compassionate, organized service and collaboration with Data, Insurance, Pharmacy, and Logistics teams to keep orders moving.

You will onboard patients, manage scheduling and documentation, and respond to questions while maintaining HIPAA compliance and accuracy in a fast-paced environment.

Qualifications

• High school diploma or equivalent.
• Active pharmacy technician registration in Texas.
• Eligibility and willingness to obtain registrations in New Jersey and other states as needed.
• Strong written and verbal communication skills with a compassionate, patient-centered approach.
• Excellent attention to detail and ability to manage a high volume of patient information, documentation, and tasks.
• Strong organizational and time-management skills with the ability to prioritize needs and proactively move work forward.
• Strong problem-solving skills and a willingness to investigate and resolve issues.
• Sound judgment and knowing when to escalate to another team.
• Ability to work independently while collaborating with patients, pharmacists, providers, and cross-functional teams.
• Ability to thrive in a fast-paced environment while maintaining accuracy.
• Proficiency with Microsoft Office Suite, Teams, and Excel.
• Familiarity with EMR systems, pharmacy platforms, or other healthcare technology.
• Knowledge of HIPAA requirements and a commitment to maintaining patient confidentiality.

Responsibilities

• Provide compassionate, accurate, and timely support to patients through calls, messages, and other channels.
• Support patient onboarding by gathering, confirming, and documenting patient, prescription, medication, allergy, insurance, and other information.
• Coordinate medication scheduling, refills, shipment arrangements, and other activities to support timely fulfillment.
• Respond to patient questions about status, scheduling, refills, shipments, and documentation requirements.
• Educate patients on the fulfillment process, shipment expectations, available resources, and what to expect next.
• Resolve patient questions and service needs within the scope of your role and follow through on commitments.
• Identify missing information or issues delaying orders and take action or escalate to appropriate teams.
• Collaborate with Data, Insurance, Pharmacy, Logistics, and other teams to move orders forward.
• Communicate professionally with patients, provider offices, pharmacists, and internal teams to resolve needs.
• Document patient interactions and account updates accurately and promptly.
• Recognize clinical questions or concerns and connect with a Pharmacist or clinical resource when needed.
• Monitor work queues and prioritize daily tasks to meet service and workflow expectations.
• Maintain accuracy while managing multiple interactions and priorities.
• Protect patient confidentiality and follow HIPAA, policies, and regulatory requirements.
